Aymeric Augustin ac37ed21b3 Deprecated TransactionMiddleware and TRANSACTIONS_MANAGED.
Replaced them with per-database options, for proper multi-db support.

Also toned down the recommendation to tie transactions to HTTP requests.
Thanks Jeremy for sharing his experience.
