Fixed #3312 -- CheckboxSelectMultiple no longer uses duplicate ID attributes for each checkbox

Regarding auto_id, CheckboxSelectMultiple is a special case. Each checkbox
gets a distinct ID, formed by appending an underscore plus the checkbox's
zero-based index.
